Halton Healthcare has been selected by Ontario Health to lead two new regional intake hubs aimed at improving access to mental health, addictions, and orthopedic care across the Central Region.
Ontario Health announced that Halton Healthcare will serve as the hub lead for both the Mental Health and Addictions Central Intake Hub and the Orthopedics Central Intake Hub – initiatives designed to streamline referrals and better coordinate services for patients and families. The hubs are intended to reduce delays, simplify system navigation, and create a clearer pathway to care as demand for services continues to grow across the region, which is home to more than 4.2 million residents.
“At Halton Healthcare, we have spent more than 13 years building strong expertise in coordinated access, and we’re proud to have a leading role in Ontario Health’s vision for expanding connected, convenient care across the Central Region,” says Melissa Farrell, President and CEO of Halton Healthcare. “This work reflects the strength of our partnerships and our shared commitment to creating a more seamless and patient-centred system.”
As hub lead, Halton Healthcare will help coordinate access across more than 49 partner health service providers, including hospital systems and community-based organizations. Collectively, these partners represent more than 650 services and programs, offering multiple specialized pathways. The coordinated approach is expected to reduce hand-offs between providers and deliver a more integrated experience for people seeking care.
The new role builds on Halton Healthcare’s existing coordinated access initiatives, including one-Link for mental health and addictions services, the Mississauga Halton Diabetes Central Intake Program, and several regional pathways recognized for innovation, quality, and scalability. By aligning how patients enter the system and strengthening collaboration among providers, the hubs aim to support a more efficient and patient-centred healthcare system.
