Ontario’s Special Investigations Unit (SIU) has concluded its review of a fatal incident in Burlington this past summer, determining no criminal charges will be laid against Halton Police officers.
The case stemmed from a June 3rd call to a home on Cornerstone Drive, where officers responded to 911 reports of a man in crisis. When they arrived, police found a man’s father dead inside the home and the 28-year-old suspect upstairs, armed with a knife. Despite repeated negotiations, the man stabbed himself in the neck before struggling with officers. A conducted energy weapon was deployed, and first aid was provided until paramedics arrived, but the man was pronounced dead at the scene.
Following a detailed investigation involving forensic analysis, witness interviews, and a review of police procedures, the SIU Director determined the officers acted lawfully and with due care under difficult circumstances. The report states the officers attempted de-escalation, called for additional resources, and provided immediate medical aid.

Ontario’s Special Investigations Unit (SIU) is looking into the death of a 28-year-old man following a police response to a disturbance early in the morning on Tuesday, June 3rd in Burlington.
At around 12:30 a.m., the Halton Regional Police Service (HRPS) were called to a residence on Cornerstone Drive where they found the man with a self-inflicted injury after he fled upstairs. Despite officers administering life-saving measures and calling EMS, he was pronounced dead at the scene.
Inside the same home, police also discovered another man dead and a woman suffering non-serious injuries, who was taken to hospital. The HRPS is conducting a separate criminal investigation.
The SIU has assigned four investigators and two forensic investigators to the case and designated three witness officials so far.
