A new mobile health van is making it easier for vulnerable residents in Halton to access the care they need – right where they are.
Operated by Support House’s Community Health Centre (CHC), the van travels across north and south Halton, connecting people to primary care services at locations like local shelters, encampments, the Churchill Neighbourhood Centre, and the Acton Youth Hub.
They also run a weekly drop-in clinic in partnership with The Salvation Army in Milton, helping residents access timely support in a familiar and accessible setting.
The mobile CHC is staffed by a multidisciplinary team that includes physicians, nurse practitioners, registered nurses, a social worker, a peer support worker, and more. They provide same-day care for urgent health concerns, chronic disease management, harm reduction support, addictions medicine, and help navigating social services. Since launching in fall 2023, the program has already served over 500 clients.
Joanna Dickinson is a nurse practitioner for the team. She says the van allows them to be in multiple spots and provide help to those who need it most.
The mobile health initiative aligns with Ontario’s broader goal of providing more connected, convenient care by expanding access through interprofessional primary care teams. Funded through the Interprofessional Primary Care Team (IPCT) initiative, the program is supported through April 2027. Dickinson notes the goal is to make this van a permanent part of the community that continues to help residents.
Referrals to the mobile team can be made by community agencies, healthcare providers, or even through emergency services – no doctor’s note needed. To learn more about the program or how to make a referral, click here.
